Eligibility
Direct Payments are funds provided by your local council following a Care Needs Assessment. They allow you to choose your own support provider and remain in control of how your support is delivered. You are eligible for a care and support needs assessment if you…
Are over 18 years old
Have physical or mental impairment or illness
Require support to achieve two or more of the outcomes below because of a physical or mental impairment or illness

Manage and maintain nutrition
Make use of community services
Make use of your home safely
Carry out caring responsibilities for a child
Maintain a habitable home environment
Maintain personal hygiene
Develop or maintain family and other personal relationships
Manage toilet needs
Access or engage in work, training, education or volunteering
Be appropriately clothed
We provide services to help you achieve outcomes one to six.
